Application Procedure

Applications are accepted throughout the year but must be received no later than February 1 for the cohort that starts in August. Applications must include ALL official college transcripts (not just your most recently attended school) and three letters of recommendation. Interviews are conducted in February and March, and decisions are made in early April for an August start. If a class does not fill, interviews will continue until July. You may apply online or print out a hard copy of the application (see below). Your personal essay must be attached to your application and must be either a PDF (.pdf) or Microsoft Word (.doc or .docx) file. Any other file extension will not be opened. Applications are also available upon request by contacting the Program Director via phone or email:

Recommendation Form

The Program requires applicants to submit three letters of reference and should be academic or work-related. You may submit these in one of three ways: 

  • Click on the Recommendation Form link. Copy and paste the URL into an email to each of your references. They can then complete and submit the form directly to the program.
  • Click on the Recommendation Form link. Print the form and hand it to your references along with a stamped, addressed envelope so that they can complete the form and mail it to the program. The envelope should be addressed to the Cytotechnology Program address shown above.
  • Your reference can also download and complete the recommendation form and send it to the program via regular mail, email or fax.

PLEASE NOTE: Three completed recommendation forms must accompany each application.

Foreign Applicants

If a candidate is basing her/his eligibility on a degree from a foreign university (i.e., outside of the United States or Canada), the official transcript or copy must first be evaluated by an ASCP-approved evaluation agency listed at the below PDF link.

Only evaluations from these listed agencies will be accepted. The official or notarized evaluation showing degree equivalency and a subject breakdown must be submitted with the application for certification examination. This document must be in our office prior to the first day of the examination period in order to determine your eligibility.

PLEASE NOTE: Official transcripts of ALL college credits must accompany completed applications. Application processing begins in December with applications accepted until March 1st.  A personal interview is also required before admission. Class selection occurs on or after April 1st for an August start. Applications will be accepted until the program is full.
